Sometimes the foreskin can become infected and then swells and causes difficulty with retraction. When it is mild, anti fungal cream can be used. But when it becomes so swollen that you cannot retract the foreskin, you need to see a urologist (doctor that specializes in treating problems of the urinary tract). You may need a circumcision to resolve the problem. This may sound scary, but in adults, it is done under anesthesia by a well trained urologist/surgeon.
So the cause may be an infection, which can sometimes (in mild cases) be treated with an anti fungal or antibiotic. But phimosis can also happen due to scarring from trauma or forcibly retracting the foreskin. Steroid cream can be used to decrease the inflammation. Again - if it is severe such as the foreskin is splitting or entirely unretractable, then circumcision is needed.
From the picture you sent, it looks like you may have some ballanitis (infection of the foreskin).
I strongly recommend you go in to be seen by a doctor where you can be given corticosteroids and an anti fungal cream and assessed for how severe the problem is. If you cannot urinate, go in to the ER immediately. If you are developing pain or increased swelling, then go in to be seen promptly.
