Suggest Treatment For Acute Appendicitis

default
Posted on Fri, 10 Oct 2014
Question: Yes I have dx of " findings of very early acute appendicitis". No fever, normal white blood count, no pain now. Have RX for Flagyl - I prefer not to use because I already have all numb toes, ball of feet and hot feet (peripheral neuropathy) that at this point I manage without meds and would not want Flagyl to potentially aggravate the peripheral neuropathy . I am allergic to Penicillin. A number of other drugs cause serious intestinal distress, to a degree that leaves me unable to work while on them - the Cycline group seems the harshest ones. I have had good results with Clindamyacin conquering a long term recurring infection I once had - is Clindamyacin acceptable for my inflamed appendix?

Brief Answer:
Clindamycin can be used instead of flagyl.

Detailed Answer:
Hi
Flagyl is used for anaerobic infection. Clindamycin capsule has activity against Gram positive bacteria, bacteroides and anaerobes. So, instead of flagyl, you can take Clindamycin.
But, Appendicitis infection needs broad spectrum antibiotic.
Along with Clindamycin, I prescribe Antibiotic like Cefuroxime tablet to my patients with similar complaints. I also give Amikacin injections for Gram negative bacteria coverage. Discuss about these medications with your treating doctor.
At this age, if appendix is inflammed, it is better to get it removed surgically. Complications like rupture are more in children and elderly.
Please consult your doctor and proceed.
Nothing to worry.
If you have any questions, I will clarify further.
Wish you good health.

Cefuroxime seems to have some kind of contraindication for the penicillin allergic people.
Would Zithromycin be an acceptable alternative in this type situation as a broad spectrum RX?
