I have gone through the single photo attached.
It appears to be balanoposthitis probably due to fungal infection.
I would have advised following:
Clean area with warm water twice daily. Apply antifungal lotion like clotrimazole .Oral antifungal like flucanazole 150 mg weekly once may be needed after discussion with your doctor.
It appears that there is problem of retracting foreskin. So you need to consult surgeon.
You may need a minor surgical procedure like slitting or circumcision.
You can get back for any clarification.
