Suggest Treatment For Abdominal Pain, Rectal Bleeding And Dizziness

Question: Hi. I have A family member suffering with a lot of abdominal pain and has many symptoms I'm trying to help her find a diagnosis. She is suffering with daily horrible rectal bleeding (burgundy color), rectal pain, rectal Fissures, stomach pain and stomach cramping, extremely dizzy, disoriented, can't stand longer than five minutes due to rectal pain, dark bloody stools, can't sleep due to pain, abnormal loose stools, occasional constipation randomly, very tired and has fevers daily around 101 or higher, chills and aches, pain and cramping and intestines with eating, feels overly full when attempts to eat, Some lab test have been performed by MD were as follows: TSH was normal, IGA was normal, tissue transglutaminase IGA was normal. Please help.

Dear patient,

If someone with your symptoms comes to my office I think about an inflammation like inflammatory bowel disease first. This is best to be examined by colonoscopy. Also in addition to the blood tests run so far I would check for inflammation signs like CRP (c-reactive inflammatory protein), ESR (sedimentation rate) and red/ white blood count (leuko/ erythrocytes).

Once diagnose is found/proven, there is effective medication available to give her relief.

For the pain she can take something known as a sitz bath. It helps in relieving the pain and inflammation. But you have to do it daily and take time. Follow this link how to take a sitz bath. (http://www.webmd.com/digestive-disorders/sitz-bath) Once it heals, then only the colonoscopy can be done. Meanwhile please take plenty of fibers as constipation is the main reason for this to get worse. You also have to see a gastroenterologist at a later time for the scopies. For the fevers she needs to take OTC Tylenol and if it is maintaining at that temperature since long, she should get a blood culture too done to check for infections. I don't see the basic blood investigations that ought to be done by now. Please check with your MD and ask him to order such investigations like CBC, ESR, CRP, Blood culture and sensitivity, and basic urinalysis with Urea and serum creatinine.
