Brief Answer:
Can be proctitis or anal fissure or hemorrhoid.

Detailed Answer:
Hello,
Thanks for choosing HealthcareMagic for your query,

As per the brief description given by you it can be proctitis or anal fissures or hemorrhoids.(Most likely related to your food habits).

Proctitis is an inflammation of the lining of the rectum. The rectum is a muscular tube that's connected to the end of your colon. Stool passes through the rectum on its way out of the body from anal aperture. Proctitis can cause rectal pain while defecating. It can be an indicator of underlying inflammatory bowel disease like ulcerative colitis etc.
An anal fissure is a small tear or crack in the lining of the anus. It may occur when passing large or hard stools. An anal fissure can cause sharp pain while passing stools but usually is accompanied with bleeding during and after bowel movements.
Hemorrhoids can cause itching and pain.
All three conditions can be accompanied by bleeding but is not always present.

A detailed physical examination by a general surgeon should be done along with digital examination of rectal and anal canal.
Proctoscopy can be done to rule out possibility of hemorrhoids.

Please tell me if you have any history of constipation?
Your diet plan?(food habits).

Brief Answer:
Its due to constipation.

Detailed Answer:
Hi again,
As i suspected most likely its because of sever constipation you are having.
Don't go for any test right now,Just follow the advice i would suggest you.

Straining a lot while passing stool can cause erosion of soft mucosa of rectum leading to bleed.

Drastic change in life style is required.
-Not exercising enough, not drinking enough water and delaying a bowel movement can worsen situation.
-Take a natural and simple diet.
-Fruits and vegetables, which contain high amounts of fiber, should be added to your diet ,Whole-grain bread, XXXXXXX oatmeal, beans and lentils can also relieve the problem so use it Instead of highly processed rice, eat brown rice, millet.
-Take Fybogel husk-Fybogel sachets and Fybogel Hi-Fibre sachets both contain the active ingredient ispaghula husk, which is a type of medicine called a bulk-forming laxative. Ispaghula is a type of fibre. It swells when it comes into contact with fluid in the gut and thus increases the volume of the faeces. It increases the bulk in your stool, an effect that helps to cause movement of the intestines. It also works by increasing the amount of water in the stool, making the stool softer and easier to pass.
-While sitting on the toilet, support the feet on a small stool in order to achieve a normal position that aids in bowel movement.
- Avoiding alcohol and caffeine.
-drink at least eight glasses of water daily. Water helps push the food through the intestines and also helps lubricate intestinal tissues. When the body is dehydrated, the intestines absorb the fluids from the food waste, which in turn leads to hardened stools.
-Can take 10ml of lactulose every night for few days to soften stools.

Follow up after a week.

Hope to have been helpful!

Wishing good health,
Thanks.
