Question: Coughing,generated from tickling in throat-4 months; exray negative; with bloating, loss of taste, tongue desensitized from burning sensation; plain water tastes salty, burping, . Throat camera negative--some loss of swallowing ability-gradually improving. Suspect it is recurrence of Miller Fisher variant of Gueon Berrie.

Miller Fisher syndrome is a rare, acquired nerve disease that is considered to be a variant of Guillain-Barré syndrome. It is characterized by abnormal muscle coordination, paralysis of the eye muscles, and absence of the tendon reflexes. From the history you have mentioned you are having Gastro esophageal reflux disease with severe gastritis because of abnormal muscle coordination.Reflux of contents of stomach back into the esophagus causes severe burning sensation(heart burn). Reflux contents irritates the esophageal mucosa causing irritant cough.The acid causes loss of taste sensation by destroying the taste buds.
I would suggest to to start taking Rabeprazole along with Levosulpiride once daily to control the reflux.Antacids should be taken with every meal to neutralise the acid.
If still not relieved Acotiamide 30mg can be taken three times a day.
