Suggest Treatment For A Kidney Stone In The Ureter

Question: I have kidney stone problem having a stone in my both right and left kidney.
the size of stones are
left --- 7.5 mm
right -- 8.25 mm , 3 mm

Left kidney stone is in ureter now and I face unbearable pain sometimes.
my creatinine is 1.06 as per latest report on last week.

1)Site of the stones in individual kidney?
2) Degree of obstruction (Hydronephrosis ) reported in Ultrasound Scanning?
3) Severity of pain in grade in scale of 1-10?
4) Is the pain associated with vomiting and fever ?

The severity of pain is 7.
once the pain is at level 7 ,I feel like vomiting.Pain increases gradually.
and almost block the urine

I will highlight the modality of treatment in detail as under

1) You have impacted stone in LT Mid ureter causing grade 1 Hydronephrosis .
In view of severe pain that you have it suggest that the stone is impacted tightly and there is infection of the renal system on Lt side This stone needs to be fragmented by endoscopic procedure URS to deobstruct the Lt kidney and to make the passage clear of obstruction to prevent further damage to the let kidney.

2)Both the stones in the Rt kidney are non obstructing stones hence you can expect them to pass down spontaneously over a period of time .Repeat Ultrasound Scanning after one month However if you get severe pain on Rt side you may need to visit your urologist and take a call depending upon the situation at that particular time .

Other option is to go for Extra Corporeal Shock Wave Lithotripsy (ESWL) to fragment the bigger stone .This is a non invasive procedure done as a out patient procedure

Any pain killer you would like to suggest me in case of pain
and medicine to fragment the stone into small pieces.

I dont want to go for surgery.

There is no medicine that can fragment or desolve Urinary stones.

The primary aim in management of stone is to save the kidney from being damaged due to obstruction .Once the obstruction is removed pain goes off automatically .

Medicine that are being prescribed for pain relief have effect only for few hours.

You are taking undue risk by not going for surgery ,of course choice is yours.

If you are not going for surgery take antibiotics like Cefotaxime twice daily along with Tomsulosin once daily .
