Suggest Treatment For HIV

Question: Hi sir
On 27th July 14 i came to know that i was infected with HIV (Tridot & Western blot confirmed) . I had started HAART ON 12th aug 14 after taking necessary checkups .At that time CD 4 Count was 145 & Viral load - 10652 copies. And i was taking
T.viraday(Efavirenz IP 600mg , Emtricitabine IP 200 mg& Tenofovir Disproxil Fumarate Tablets IP 300mg ) & T.Matilda Forte daily once .
All the test done in Ongole , Ap
Again i had consulted in Tuticorin Medical college on 15th Sep 14. While checking here my CD4 count was - 298 . Here they had advised me to stop the tablet which i taken earlier . They had given (T. Efavirenz IP 600mg ) (T.Lamivudine and Tenofovir Disoproxil Fumarte IP 300mg /300mg ) . So plz suggest me which medicine i can continue . And is there any chance for IVF .

Thanks for posting your query.

Certain points yet to be cleared.
1. You had mentioned that you are HIV positive, tested by Tridot and WB. I would like to know whether it is HIV 1 infection or HIV 2 infection or a both HIV 1&2.
2. Your first regimen started by a Private specialist. Then why you went to Govt. ART center?

There is not much of difference in the efficacy of both ART combinations (Tenofovir+Emtricitabin+Efavirenz and Tenofovir+Lamivudine+Efavirenz) put forth to you. In Govt. ART centers the 2nd combination only available and not the first one. But the first one is slightly better and costlier than the 2nd one.
If you have HIV 2 or HIV 1&2 then these both combinations are not the right choice, as Efavirenz has no effect on HIV 2 infection.

The viral load and the CD4 count are not so compatible with the first test. So only I am asking for the HIV typing. When the CD4 count was below 150, then the viral load would be expected to higher considerably.
Sometimes it happened to have such difference in CD4 count in different labs. That may be due to the difference in interpretation or due to recent opportunistic infection and recovery etc.
If you are able to afford in the long run, the first combination ART is preferred than the second one.
After your viral load controlled to undetectable level, with sperm wash techniques, assisted reproductive techniques are possible to have a baby. You need not even sort for IVF. IUI can be tried if your partner has no problem in her reproductive function.

Dear sir

Thank you for your reply sir , my reply according to your point wise
1.I was infected by HIV 1 Only , in report it shows ( HIV 1 - Reactive & HIV 2 - Not Reactive )
2.Friends told that in Govt ART centre they are proving same medicine & It is the best one wen compare to Private and cost wise .But till now i had not take Govt ART medicine .
Can i able to know how much time ( any timeline ) it takes to detect the viral load undetectable . (There is some problem in my XXXXXXX to upload all the Reports so i failed to upload it )
Ineed your appointment for further check up can u able to suggest plz .
Is there any serious side affects in (T.Viraday )when compare to (Govt ART Provided Tablets )

Hi,
Welcome once again.

If you are infected only with HIV 1 then the ART given to you is ideal. Nothing to worry.
It is not true that Government drugs are better or Government schedule is better. Only thing is the ART doctors are well trained and exposed to HIV medicine very well. If your private doctor is equally well versed in handling HIV cases and updating himself then and there, and if you are able to afford for these drugs in the long run then taking the medicine from the private source is always better as you and your doctor are able to take any decisions easily whereas there are many drawbacks (which can not be enumerated in this forum) are there to get followed up in a Govt. ART center.
Following an effective ART regimen the viral load has to come undetectable within 24 weeks if the adherence is very good. You can contact me through the address and phone number given in HCM site.
