Suggest Treatmemt For OCD,anxiety And Severe Depression

Question: Hello, my name is XXXX and I have an adult son with OCD, anxiety and severe depression. He has had many years of therapy and on different medications. He knows about cognitive therapy but is too depressed to try. He has over used his medications and has an addiction. He is a R.N. and lost his last job because he took a patent meds on the job one night. He sleeps 24/7 and only gets out of bed when I get him up to work for his new job. Nothing will get him up and stay awake. He is on his seventh job within the last seven years. I’m grasping for help and I fear the very worst. He is single and lives alone with his dog. This is the most debilitating illness, he has no life. Only the bed. The world is spinning without him This may be out of your field but I’m praying for help. I just know that if he could get someone to help him he may have a chance of some kind of normal life. He suffers severely. I’m praying for a miracle, Thank you.

no he is not using recreational drugs. His problem started around six or seven years ago when his doctor put him on retalin. It work very well for him but he started self medicate and gradually up his dosage and than ran into over use of Retalin. He was forced to get get off this medicine and he lost his job. He has from time to time taken pain medication when he felt he needed it to help him get through a very bad day. This is only when he could get something. I do know this last time he took a patients oxycontin on his shift and this is why he lost his last job. The nursing board had him to be interviewed by a specialist and she said she did not feel he needed to into inpatient clinic. She suggest that he go to Narcotics Anonymous and get drug tested and a probation period. He is still waiting for the ruling. He is now working with a small hospital. His current medications are clomipraim or clorazepan, provigil, abilify and another anti depression medicine something like busbar.

This all started when he was around 17 years old and the sleeping has gotten exceeding worse. It has come and go but for the last few years it has become excessive. I do not believe the medications have been all of his problem. It has become a disease of depression and years of habit. I know that if he can not get better he will die. I do not know how to help him anymore. I already do everything else for him.

Sincere apologies for the delay as there was some technical problem on my side.
I understand that your son is having an OCD with depression and he certainly not doing good.
At the outset, i would like to let you know that OCD is a difficult illness to treat. But it needs to be treated intensively especially in cases like your son where the debility is too high. I have gone through the medicines he is on.
For my patients , i prefer using high doses of fluoxetine or fluvoxamine and they show pretty good results. it seems your son has not been tried on these drugs and with your psychiatrist you can consider putting him on these drugs. Further addiding risperidone is also a good option to augment the current treatment.
If these things do not improve then DBS (deep brain stimulation) is a neurosurgical option which can help him.
Also in regular course he should regularly attend the session s of cognitive behavioural therapy. All these options will help him but he needs to be consistent over them.
