Suggest Remedy For Strep Infection In Genital Area

Question: I have a five month old baby boy.
He had a moderate strep infection in his diaper area about a week and a half ago...I got some antibiotic cream from his doctor. Unfortunatley, I missed giving him several doses, I think the infection might still be there. He is acting very fussy, I am scared...how serious is this?

I am taking him back to dr. tomorrow, he was born at 30 weeks and had sepsis in the NICU, is it possible he has it again?

1. What was the birth weight and what is the current weight?
2. How is the attaining of developmental milestones?
3. Is there any high grade fever above 101.5F?
4. Is there any color change in the skin?
5. Is there any history of recurrent serious infections in the past?
6. Was he hospitalized anytime after the newborn period?

Will tell you the most common signs of sepsis for early recognition and also to put you at ease if these are not there -
Crying more than usual
Cannot be comforted
Sleepy or hard to wake up
Does not want to play
Fussy for long periods of time - I mean unusually longer than his normal.
Not eating or sucking well
Rash
An area of redness, swelling, warmth or pain
Trouble breathing [fast or slow breathing, coughing, wheezing (making a whistling sound)
Stuffy nose or running nose
Diarrhea/ Vomiting
Not as many wet diapers as usual

I do not think that the original streptococcal rash is responsible for this kind of symptoms. But hereafter I suggest you to follow up the medications scrupulously to avoid agitation.

His birth weight was 3 lbs 2 oz and he know weighs about 14 lbs
Yes, he has met milestones for his adjusted age
No fever
I do think that there is redness in his diaper area around end of penis
Only other infection he has had was in NICU, that was staph, he had been in NICU for several weeks before he got it
He was hospitalized for apnea monitor going off but no additional treatment was needed

He is definatley crying more than ususal and being a little less playful and wanting to interact with me a little less...does this sound like it may be sepsis?

If it is not sepsis, is this type of infection likely to have a long lasting effect? Cause autism?

Hi...Coming to your queries -

1. Yes this sounds like sepsis - I suggest you get him evaluated by a pediatrician immediately. I am saying this keeping in mind the stormy newborn period in consideration.

2. This can be a simple viral illness too. Do not worry. Most important thing is his urine output. If this is normal and his feeding is normal - you need not be worried.

3. Autism has many causes and reasons, one among which is sepsis and even stormy newborn course. But it is too early to comment on that or confirm.

I took him back to dr. today. They said that there is no infection present in diaper area. They checked his temp, breathing, nose, eyes, ear and throat and said that perhaps fussiness is a cold that is coming on.

The infection that he has was Strep A? Which they said is on the skin all the time and doesn't really require treatment? Does that sound right? Is it possible that the infection would be no longer visable on penis because it's in bloodstream?

Are any other tests needed?

Hi...
1. Streptococcus is a ubiquitous organism, found literally everywhere. What was told in doctor's clinic is true. They exist normally on skin too. It doesn't require treatment as such.

2. If the penis is healing well...then its unlikely that the infection is still lingering in blood stream. If there's blood stream infection, the baby would be sick and would have warranted hospitalization from doctor's office itself.

3. As I suggestion in my previous interaction, look for the red flag signs and symptoms. If those are not there, no need for any other tests.

Thank you, I am just a nervous new mom...the only symptom that concerns me is that he is eating less but they checked him out pretty thoroughly.
