Question: Back pain in lower back which is severe, no radiation very often...What could this be?

Spine disc herniation.

Hello!

Thank you for the query.

Lower back pain is mostly caused by spine disc problem in the lumbar area. Especially if it sometimes (or even rarely, but still) radiate to the leg. The pain can get more intense after physical activity or lifting heavy objects.

In some cases, back pain can be caused by kidney infection, however usually lower abdominal pain, frequent urinating or burning while urinating can be present as well.

I suggest you to have spine MRI (or CT) and consult orthopedist.
