Brief Answer:
Long list of causes, but focus on other symptoms

Detailed Answer:
Hello,
Thank you for posting your query to HCM,

Persistent and intractable hiccups frequently are symptom of underlying disease or injury, but may be idiopathic or induced by psychological factors.
There are more than 100 diseases and etiologic agents have been shown to induce persistent and intractable hiccups including gastrointestinal and esophageal problems, cardiac problems, abdominal, pulmonary, central nervous system, metabolic, medications, psychological, ect, ect. Parasites are not on this long list.

I will list the labs that would be as initial testing to diagnose the cause of it, because its treatment will be to treat that: complete blood count; serum electrolytes; blood urea nitrogen and creatinine; electrocardiogram; chest x-ray.
He has done some of them, and I hope the others will help rule out that cause.

I think you should maybe focus in the system that he has other symptoms from...like in your case there is cough, and chocking...so should maybe focus on the lungs and upper airways. If he has other symptoms, the doctor should go with the labs following those symptoms.

Hope this is helpful. Let me know if you have other questions. I would be happy to answer.

Take care,
Dr. Papaqako.
