Question: I'm having really bad, watery diarrhea after taking 4doses of amoxicillin-clav after multiple cat bits on my hand. Threw up violently mon nite hours after first dose. ER Next day gave drug to stop neausy and bit shot of cefTriaxone and dr put me on doxycycline. 2days later everything just going thru me with major gurgling stomach. Saltines I can barely swallow mouth so dry, plain Greek yogurt and a banana over last 2 days. Drinking lots of water. Tried Imodium last nite/ today per urgent care call last nite. How do I stop this other than stopping all food! At least cat bites on hand look MUCH better as they were bad.

Brief Answer:
Possible clostridium dificille infection

Detailed Answer:
Thank you for contacting HCM with your health care concerns

I am sorry to hear that you have been having so much troubles with your current course of treatment for cat scratches. Your initial antibiotic amox/clav is well known to cause diarrhea. Because it is so good at killing bacteria is may also kill the positive or good bacteria in your colon. In you case I think you have a new super infection known as clostridium difficile. This is when an antibiotic kills off the good bacteria and then there is an overgrowth of bad bacteria. It causes severe diarrhea.

Unfortunately you need to go back to your medical provider and have a stool sample sent for the clostridium difficile toxin. If it is positive then you will need to be on a antibiotic called metronidazole. Once you have taken it for 2 weeks the infection should be resolved.

To help right now stop all milk and milk products including yogurt. Drink plenty of fluids and use the imodium sparingly as it may actually perpetuate the clostridium infection

I hope this answers your question. Please contact us again if you have any further questions

Thank you so much for the fast and specific answer. I thought that might be the answer too. I did go back to my urgent care center but they did not want to give me metronidazole yet. Instead they told me to stop all antibiotics and not finish the doxycycline as my hand was looking better. So only 2 days on amox-clav and 1 1/2 days on the doxy. They said to and said to take 'Align ' probiotics daily to see how things go.

My question is does that sound good and how long should I wait if the diarrhea doesn't stop?

I ate a banana today, a few crackers (neither stayed more than 30 min). Now tonight trying some chicken/ rice soup but stomach already making ominous noises with just 1/2 a bowl. No pain or fever.

Thank you again, ?JoAnne

Brief Answer:
I disagree with the plan

Detailed Answer:
I am surprised that they did not do a test for C. difficile. It is a quick and easy test and instead of just wait and see you would have your answer by this evening. I recommend by passing the urgent care and go directly to your primary care doctor and express your concern. State you concerns firmly.

My recommendation for you is to just do the BRAT diet, Bananas, rice apple sauce and toast. Keep your food intact to bland foods. The main thing is to maintain fluids. You can fast not eat and not run into any problems but fluids will need to be replaced.

I hope this answers your question

Thank you so much.. Your fast response and helpful advance was worth every XXXXXXX and more. I called my primary doctor today and after a lot of resistance with the receptionist I got the nurse, who then checked with my doctor who did approve the test. I love my doc, she's great but so busy they told me it would be a week to see her and get referring me back to urgent care (which they own and while also very nice are MUCH more expensive).

Finally after another really miserable night with even a banana and later Apple sauce running thru me in 15-30 min, I finally started to feel better after plain rice w/salt at 10am that stayed in me almost 3hrs. Then more rice at 3pm that is good 4hr later.

Thank you again. I'll definitely remember this as a future option for solid and reasonable medical input! (Truly more grateful than you know as I live alone with no one really local to ask for advice)

Regards, XXXXXXX Halligan
