Question: 53 year old female nurse with medial knee pain. mri showed deformity in tibial articulating surface of posterior horn of medial meniscus. marked focal thinning of patellar cartilage, full thickness chondral deffect to pattelar facet. multiple moderate osteochondral lesion on patellar surface. moderate patellofemoral osteo changes, with ocd foci. small suprapatellar effusion. cannot exclude focal tibial rticulaticulating surface tear of medial meniscus, area of root. ongoing pain since july, had cortisone injection 2 wks ago with no releif

Thanks for posting your query.
The findings of your MRI are suggestive of inflammatory changes in the knee joint.
Physiotherapy like ultrasound and TENS will help, along with knee exercises
For level of osteoporosis DEXA scan can be done to start proper medications.
Life style changes will help in reducing pain which include,
LOAD REDUCTION: Protecting the joints from excessive load may slow down the rate of cartilage loss. It is also effective in relieving pain. Avoiding activities like climbing stairs and using a walking stick are worthwhile.
ACTIVITY MODIFICATION –
USING CANES: Assistive devices can make it easier to go about your day without stressing your painful joint. A cane may take weight off your knee or hip as you walk. Carry the cane in the hand opposite the leg that hurts.
You can also use a knee support or knee brace for relief.
VISCOSUPPLEMENTATION: Hyaluronic acid injections given into the joint in prescribed dosages will give good short term pain relief ranging from 1-2 years. You can take them after consulting an orthopaedic surgeon.

Many of my patients do get relief by yoga - pranayama and XXXXXXX XXXXXXX You can start with these under the guidance of an expert.
If there is severe destruction of menisci then arthroscopic shaving can be done.

i also have rheumatoid arthritis, and dr said an 85% i have a meniscus tear, if no tear, why so much pain? thank you.

The pain is due to the inflammation and degenerative changes and reduced joint lubricants. Even if there is a tear a conservative treatment can help you. Diagnostic arthroscopy can be scheduled to detect and repair the tear.
