Question: Hi myself Gladson having blood pressure ranging from 140/90 on an average since last 3 years, initial when it was detected my blood pressure was at 190/110. I am having Nebistar H 5 mg since last 3 years on a daily basis after having my breakfast, however, in last six months my blood pressure has gone bit more since I am consuming alcohol almost on a daily basis due to stress at my office as well as at home. Doctors have advised me to take one additional tablet CETAN cilnidipine 10 mg tablet during night but still after taking one more tablet my blood pressure has not gone down.

My current age is 38 and sperm count has gone to zero in last 3 years. I am not having any kids so far with my 2nd wife in last 5 years, however, I had 1 son from my ex-wife ten years ago. Though I am having alcohol since my first wife left me alone. Due to this depression I am having alcohol almost on a daily basis in last 5 years, please advise which should be best medicine for me to cure my blood pressure without having any impact on my sexual life and advise me if there are any chances to increase the sperm count since my wife is really worried lot since we are not having any kids so fat in last 5 years

Brief Answer:
Blood pressure control

Detailed Answer:
Hello

Thanks for the query

Here are my suggestions on how to have adequate control of blood pressure

1. Continue taking the medicines prescribed, they are wonderful drugs for blood pressure
2. Reduce your salt intake to less than 2g/day. Avoid salty food like salted nuts, pickles, etc
3. Exercise everyday for 30 min atleast. Cardivascular exercises are the best, like walking, jogging, cycling, etc
4. Avoid foods rich in fat
5. Loose weight, weight loss itself can help reduce blood pressure

Tips to increase sperm count

1. Avoid keeping hot objects like laptop on your lap
2. Take cold water showers daily
3. Wear boxer type of underwear
4. Avoid masturbation before coitus and during the ovulation period of your wife
5. Loose weight
6. Avoid smoking
7. Limit alcohol to just twice a week,
8. Seek professional help for infertility

I hope I was of help, if you have any further queries please get back to me

Thanks very much Doctor, I follow almost everything what you mentioned in your reply since I visited multiple Doctors across XXXXXXX in last 5 years but I am having major issues in getting sleep. I fully understand it is mainly due to Blood Pressure. I hardly get sleep for 5-6 hours that too after drinking alcohol of 6 pegs large (60 ml), which is not at all good for my liver and other important parts. I spend so much time and money and at the same time taking very big risk of life.
Looks like I am having sleeplessness or sleep disorder, I am not sure I may be having Insomnia. I know those who are having trouble in sleeping sometimes turn to sleeping pills, which can help when used occasionally but may lead to substance dependency or addiction if used regularly for an extended period.
Considering the above points, can you please guide me as I am now helpless after visiting so many Doctors I am not finding the right solutions. Infact there were several test done so far related heart (2 D) and kidney and lipid profile, etc. but looks like the exact reason for not getting sleep is not known yet.

Brief Answer:
Amitryptaline is not ideal for sleep

Detailed Answer:
Hello

Thanks for getting back

Amitryptalline is not a great drug to make one fall asleep. I prefer zolpidem or clanazepam, higher doses are usually required in patients who drink regularly as sufficient effect will be suppressed because of daily alcohol use.
Alcohol itself is one of the major causes of disturbed, inadeqaute sleep. It prevents you from getting deep sleep.

I strongly suggest you to taper and stop your alcohol habits as this one thing can solve most of your issues like sleep disorder, weight issues, blood pressure etc. Seek professional help, attempting to do it on your own might be difficult.
