Question: Dear Doctor,
wish you a very happy new year!
i am a 30 yr old XXXXXXX male and an airline pilot, and i recently got diagnosed(quite by chance) with lattice degeneration with atropic holes in both my eyes, also posterior vitreous detachment and i see a lot of floaters in both my eyes . i just want to know how will it affect my job, and is a laser photocoagulation a compulsory treatment in this case? and what side effects does this treatment have? will it affect my visual field? I also see a white dot (or sometimes multiple white dots/balls) move in a circular motion along the circumference of my peripheral vision. its not a flash of light..,i am not diabetic, i have cylindrical power (-0.5 both eyes) and my ocular pressure is normal (18,19).
i see cloud like wave formations when i come from bright to low light/darkness which die out in a few seconds, this phenomenon also occurs when i close my eyes (its more like a kaleidoscope) also a strobe light kind of effect (the kind of light used for an EEG TEST) in the dark.also my tolerence to bright light has reduced, these are all recent developments, or maybe i have started noticing them since my diagnosis. otherwise my vision is normal, even with the amsler grid.
What do i do with this condition? how do i proceed with a treatment? my opthalmologist has suggested to wait and watch and go for regular check ups. is the condition going to get worse? please help .

I reviewed your history.
It seems from the history that you are having symptoms of floaters and flashes of light and your eye doctor has made a diagnosis of lattice degeneration with atrophic holes in retina and posterior vitreous detachment.

Lattice degeneration by itself is a benign condition and it does not require treatment other than regular follow up. But since you are having symptoms of floaters and posterior vitreous detachment I would advice you to go for prophylactic laser photocoagulation to prevent risk of development of complications like retinal detachment.
Laser photocoagulation is a safe procedure and it is a simple office procedure. It will not cause any problems. This will not affect vision.
Hence considering your profession, symptoms and co existing posterior vitreous detachment I will prefer laser treatment.
The condition requires regular follow up as progression is known to occur particularly in cases where prophylactic laser is not done.

Where should i get this procedure done? And i am concerned whether this procedure will afftec my visual field because as you know , for pilots the eye exams are very detailed. and does this procedure reduce the risk of RD?

Laser procedure is a simple office procedure and it can be done by ophthalmologist who is trained in laser or preferably by a retina specialist.
This procedure will not usually affect the visual field, hence no need to be worried.

The procedure will reduce the risk of retinal detachment.

Hence I advice you to consult retina specialist for further management.
