Question: Hi Sir

1. My baby is having persistent loose motions past 20 days. first time when they came doctor asked us to give probiotic endrogermina.. zinc and ziprax (an antibiotic).also he showed lactose intolerance so changed his milk to casein milk....loose motions stopped after 5 days...2nd time again doctor prescribed same treatment...changed milk to soya that is zerolac... now again it appeared 3rd time. he is currently on zerolac that is soy milk.
i had his stool test done....ph is 6.0 acidic...semi digested with mucus...pus cells...3 4 /pf. rest all nil.
Currently he is on endrogermina..zinc and pedialite. i am worried to start antibiotics for him 3rd time.

2. Can i breastfeed if baby has diarrhea as baby is lactose intolerant. what should i avoid eating.

3. can i continue supplements like Vitamin D and zincovit if baby is having diarrhea?

4. Are loose motions because of teething?

5. He is now very weak...so doctor suggested iron after 1 week when loose motion stops?is it safe?

6. His bones now make noise when i try to exercise him...does he need any calcium supplement.his appetite is very low...being 6 month baby...only 300 ml a day soya milk. i am working so i breastfeed himm only t night.

7. he does not take solids...he gaggles. please suggest...what should i give him? Also what should i give him in loose motions state.

8. Can i give nestum rice to my lactose intolerant baby? i think cerelac contains milk.

9. diarrhea makes his anus veru rashy...and painful. he cries a lot after pooping snd if i was it. please suggest treatment. earlier i use seridorm mixi and shield ointlment but does not help.

Brief Answer:
Do not worry

Detailed Answer:
Hi
Welcome to the HCM
I have gone through your question and the report attached. I understand your concerns but don't worry. Infants show great variations in colour, consistency and frequency of poops. The reports of your baby are near normal and doesn't need any antibiotics. Some mucus and few pus cells can occur even in viral gastroenteritis episode which are quite common in this age group.

Answers to your questions are as follows:
1. Continue him on lactose free diet only if the frequency is high and the consistency is very watery. Give Racecadotril granules (10g) thrice daily for decreasing the frequency along with Zinc supplement such as Syp ZnD or Zinconia.
2. Give breastfeeding and complementary feeds if frequency is less than 10 times with semi formed poops. Avoid sugars and sweetened juices. Give bananas. Give oral rehydration therapy such as lemonade, buttermilk, ORS to prevent dehydration.
3. Continue Vitamin D. Hold Zincovit.
4. These episodes occur due infection from mouthing unhygienic items which happens in this age. So, keeping a good clean surroundings is important.
5. Start iron syrups after 7 days of complete recovery.
6. You may start calcium supplement such as Osteocalcium along with vitamin D.
7. Hold lactose containing products if frequency very high.
8. For rashes, use rashfree ointment available at medical stores.

Hopefully it will be of help.
Take care

Hi doctor; So viral gasto enteritis resolves without anti biotics? How long it take to resolve it completely? Why is infection coming again and again?
Lemonade without Sugar?
Calcium can be started now or after recovery?
Could you please let me know what lactose containing items should i stop? do you mean milk and sugar? can i have tea?
Could you suggest any ointment as normal diaper rash cream seems to be more itchy to my son.

Brief Answer:
No need for lactose restriction

Detailed Answer:
Your baby's health seems to be perfectly fine. This is a normal frequency of poops. It's normal for infants poops to have offensive smell and undigested particles. It happens due to immature gut function which gradually improves as the baby grows. The weight gain is good.
I would recommend you to start him on normal diet including lactose as well. No need for lactose restriction. You may give baby foods as per his choice including nesting/eastum.
Start zinc supplement such as ZnD or Zinconia (5 ml once daily for next 14 days). It gives immunity against any further diarrhoea episodes. Also, start iron supplement such as Ferronia XT ( 2.5 ML twice daily). This will improve his appetite further.

Hopefully it will be of help.
Take care
