Brief Answer:
Check for core temperature and response to treatment

Detailed Answer:
Hi,
Thanks for asking.
I am Dr. Prakash HM and I will be answering your query.
Based on your query, my opinion is as follows:

1. Please check his core temperature. If its above 39 degrees celsius, he will require hospitalization.
2. Do follow up with blood pressure and check if its falling or not.
3. If the blood pressure is not improving, he will require intravenous fluids. At that time, he will require hospitalization.
4. Use ice cold water sponging, ice packs and cooler temperature areas to help in rapid cooling. Improve his hydration with plenty of cold water. Do not overfeed and during vomiting, put him into stable position to avoid any aspiration into lungs. If persistent, may need to go to doctor.
5. Fluid balance management and blood pressure evaluation along with continuous monitoring of core temperature is essential. If all are improving and temperature is falling, not to worry.

If he is developing any delirium or confusions or his reaction response is slow, better to go to emergency for management.

Hope it helps.
