Brief Answer:
Go to emergency right now

Detailed Answer:
Hi, thank you for using Healthcare Magic. This person needs to be seen at emergency right now. There are many possible causes, many of which can be serious. Older people can have a severe infection that presents only with confusion and not the typical symptoms of an infection. Problems with oxygenation such as a lung problem or blood disorder can be causing it. A stroke is still possible.

Metabolic problems are also possible. These could result from extremely high blood sugar, liver problems, abnormalities in electrolytes, and many other things. A recent heart attack is also possible.

He needs immediate evaluation at emergency to determine a cause and to start appropriate treatment. Bring him to emergency or call 911.

Hope this answers your query. If you have further questions, I would be happy to answer them.
