Brief Answer:
It is often decision of clinician

Detailed Answer:
Dear XXXXXXX
Thanks for using Healthcare magic.

The dose of librium is provisional on first day of alcohol withdrawal and than it is titrated as per need of the patient. For a moderate to severe drinker 100 mg or more of librium is started and every day there is reduction of 10-15 mg per day. Some patient may not tolerate 100 mg and even less can be startedd like 75 mg or 60 mg as per the decision of treating clinician.

The only thing standard about alcohol withdrawal treatment is reduction of librium dose every day and stopping it in 7-10 days time period.

I hope this answers you.
