Question: Compare effectiveness and cost to treat neuropathy for cervical spine stenosis, kyphosis, and flattening of spinal cord (given same dosage, qty.) between Neurontin, Gabapentin and NerveRenew?

Thank you for contacting HCM with your health care concerns

You have neuropathy from spinal stenosis and cord compression and you want to know what would be the best treatment options for you. You are currently on tramadol which is a good general pain relieving medication. If this works then that is the best medicine for you.

Gabapentin and Neurontin are the same medication. This medication works by trying to stabilize nerve endings to reduce pain. In my experience the amount of pain relief is limited. Another similar medication is Lyrica which again is like gabapentin and works the exact same way but may help other better than the gabapentin.

Your last choice is a nutritional supplement Nerve Renew. As with all supplements these are not studied with proper placebo controlled studies. Therefore I am suspect to any claims these supplements make. My advice here is not to take them as more than likely you are wasting your money.

In summary
Stay with tramadol if this works to control pain
Gabapentin and Neurontin same medication and limited improvement but worth a trial.
Lyrica optional maybe better than gabapentin also worth a trial
NerveRenew more than likely worthless

Can I take together, at the same time, 50mg Tramadol and 50mg Gabapentin? If so, what should be the daily dose of each medication? Is it a good strategy, after a period of weeks, to then phase out either the Tramadol or the Gabapentin and then phase in one or the other? My objective is how to determine the best medication for me. I need an effective strategy. Thanks for your help.

Detailed Answer:
You can use both the gabapentin and tramadol together. I would recommend 50 mg of tramadol daily and 100mg three times daily of gabapentin. Your gabapentin dose can vary dramatically. The usual dose is 300mg three times daily and sometimes up to 600 and 900 mg three times daily. We usually titrate to pain control and amount of side effects.

To wean from tramadol you would decrease by one pill daily and then 1/2 pill daily until you can stop. There is no weaning from gabapentin as there is no withdrawal problems with it. Tramadol may have some withdrawals which would include diarrhea, abdominal cramping, goose flesh or goose bumps and increased pain.

The daily dose of Tramadol can be from 50 to 300mg daily

With gabapentin as mentioned above 100mg three times daily to 900mg three times daily
