Question: I have H pylori and have been taking clarithromyc (500 mg) and metronidazole (500 mg) twice daily and nortriptylin (25 mg) and omeprazole (20 mg) once daily.....these combinations of antibiotic when taken cause a increase and irregular heart beat....wondering if there is some alternative to these antibiotics..

I appreciate your concern

The Helicobacter pylori eradication therapy that you have described is not up to the mark. If you are not penicillin allergic then the 1st line therapy is clarithromycin (500 mg) and amoxicillin (1 g) both twice daily in combination with lansoprazole 30 mg twice a day. This regimen should be continued for 14 days. After completion, one month of lansoprazole 30 mg once a day should be used. The PPI omeprazole is prescribed in very low dose. It should be at least 40 mg once if not more. Nortriptylin (25 mg) has no role in eradication of H. Pylori.

You may discuss these options with your doctor and apprise him of my opinion.
