Question: In my abnormal stress test result is positive for reversible myocardial ischaemia. HDL-31 LDL-118 Blood Sugar 97 Age 58

A positive stress test is an indication for further test...there is strong possibility of Coronary Artery Disease
     You must go for an Angiogram... It is the next step investigation. It will detect blocks – its presence, location, extent, its severity and feasibility of intervention. Also the left ventricle (lower heart chamber) and its function is assessed – wall motion, ejection fraction, pressure, saturation are all documented. It is the only way to directly ‘see’ the block / it is the gold standard against which all other investigations are compared.
     The aim of any investigation is to modify the treatment, based on the result. Intervention – dilatation with balloon and placing a stent (Angioplasty) – is done at or after the procedure – to ensure patency of the vessel. If it is not technically feasible, a CABG (Surgery - Coronary Artery Bypass Graft) may be necessary.

Your LDL cholesterol is high.
LDL (& non-HDL) is bad cholesterol – lower, the better.
In the presence of ischemia, 70 should be the aim.
Take a statin straight away –because of positive stress test ..
Keep your weight to normal / even a little less
     Quit smoking, if you are a smoker
     Do regular EXERCISE – say walking or jogging - 30 minutes a day / at least 5 days in a week. Isometric exercise (like bull worker) is bad.
     Healthy FOOD is a must. Avoid saturated fats – coconut oil, palm oil, butter, ghee, full cream milk...Avoid junk foods in particular – French fries have palm oil / Pizza has cheese / Indian sweets are made in ghee.../ Change to Poly-un-saturated oil – like sun flower oil. / Include Mono-un-saturated oil – Olive oil.
Repeat the test in 6 weeks.
