Should Prednisone, Methotrexate And Rituxan Be Taken For C-ANCA Associated Vasculitis, Temporary Vision Loss And Tinnitus?

Question: I have C-ANCA associated vaculitis and ÷ PR3 antibody; musculoskeletal symptoms under control with prednisone and methotrexate. However, lost vision temporarily for under 15 m. In L eye.Now. incessant tinnititus ,poor stamina, poor feeling of well being. Dx 9/16. Should I pursue Tocilzumab therapy; rheumatologist has offered Rituxan infusions; same drug? Symptoms seem to be giant cell arteritis.

Hi,

In my clinical experience, the combination of Prednisone and Methotrexate is given as long as clinically possible.

Tocilzumab efficacy: There have been some isolated case studies of single patients, suggesting that it has worked (caused remission or disease free interval) in ANCA-vasculitis.

Rituximab and Tocilizumab are newer treatment modalities and not devoid of side effects. Most important side effects are allergic reactions. Therefore these are started only when really necessary. Rituximab and Tocilizumab belong to the same group of medicines but are different in their profile. Rituximab has been studied in more detail for vasculitis and has shown promising results in studies.
