Started three days ago and yes recent change in diapers and yes itchy and I think painful for him and sometimes he gets hot but temperature is normal . Using diaper rash cream but doesn’t help

Thank you for the details given.

Seeing the picture and clinical background, this is probably a contact dermatitis- an allergic reaction of skin to some trigger, may be the new diaper.

This can be treated with Acetaminophen to relieve pain, an anti-histamine like Cetirizine to relieve itching and a topical steroid to control the reaction.

You can give him Acetaminophen suspension(160mg/5mL) 5ml 3 times daily and Cetirizine syrup(5mg/5mL) 2.5mL once daily for 3 days(assuming his weight to be around 10 kg). Also, apply 1% Hydrocortisone cream there 3 times daily for 5 days. These drugs are available over the counter.

If not improving in 3 days or if he develops fever, kindly take him to your doctor, as this might need reconsidering the diagnosis or treatment with more potent prescription drugs. Meanwhile, you can apply the diaper cream in between to soothe the area. Avoid using the same nappy brand further.
