Dear Doctor My wife is 69 year old and normal built. Oflate she is getting pain in the right knee leftside and burning sensation. stiff legs after sitting for long time to get up.subsides after a few steps.There is no swelling or inflamation in the legs. here xray, orthoapaedic says her knees are not deteriorated. enclosed herewith the x ray.No damage.
She is diabetic since 2008 .No insulin. She takes glycomet 500 mg twice .her sugar levels are always normal.HBA1 c is always below 6 in all these years.She applies some ayurvedic oils and apply diclofenac gel .she is taking calcium since many years.Now she is taking citracal. She has taken cholocalciferal alomost 3 times during the last 2 years. If pain and burning is more she takes aceleophenac or ultracet tablets.
I am wondering whether her case is an orthoapedic or a neurological.Kindly suggest how best can she manage this problem.

Firstly xray is not normal. There are early changes of Osteoarthritis of the knees with joint space narrowing. This is the cause of her stiffness and pain. Applying warm packs will help with relief. Avoid bending knee much. Physiotherapy will also help.
Regarding the burning sensation, it is due to nerve damage from Diabetes. She can benefit from Pregabalin medicine and must keep up with good blood sugar control.
A Neurologist will be able to help with the pain management better because of the Diabetes.
