Question: My wife is 77 years old and overweight. Last evening suddenly she became confused and somewhat unresponsive to questions for 3 hours and suddenly came back to normal, and hand shaking occasionally.

I have gone through your query and here is my opinion.

As per the history narrated by you,it seems that she had a Transient ischaemic attack or a minor stroke.
She has one of the major risk factor in the form of morbid obesity.
Please share,if she has co morbidity like hypertension,diabetes or heart disease.
Is she on any drugs for any ailment ?

She needs to be evaluated by a neurologist who will go for work up which includes ECG,Echo,carotid Doppler,EEG and a brain scan in order to rule out any interruption in blood supply of brain.
Blood work up in the form of blood sugar levels,thyroid profile,blood counts,lipid profile and serum electrolytes would also be procured to ascertain the cause.
