Question: My wife had a root canal done after the 15 year old amalgam filled tooth broke ( near the joint with another tooth) which exposed the original filling.

We have finished the second sitting where the temporary cement has been taken out and pre molar has been filled with some white substance. Before doing the filing the doctor said we will go ahead without a fiber post, but post the filling he told us we would do a fiber post .
Question: Can a fiber post be done "after" doing the filling for a root canal in the second sitting ( since it would involve more drilling on the filling ?)

Hii first of all thank you for approaching us with your query. I read through your query. I must say that fibre post can be done once the filling of rootcanal is done. Only a little of the rootcanal filling will be removed(from the top part) slowly for creating the space for fibre post. Its usual technique.So nothing to be worried in this treatment. From what I had read elsewhere, creating a post would "weaken roots, by removing dentin", but "a posted canal is stronger than an empty canal".

Now how can there be dentin loss if as you say only a part of the filling will be removed ?
Further, short posts are more likely to fracture and "retention seems adequate at 2/3rd root length".

Does this mean a useful post will probably need drilling beyond the depth of the filling ?

Thank you for your follow-up query.

I must say that full dentin won't be removed. Only a little dentin will be removed as some part of the dentin should be left behind around the post as it may act as a shock absorber for the stresses applied on the post while biting...

Next regarding the length, 4-5 mm of the root canal filling material will be remaining in the root from the tip. Rest of the material will be removed for the post space. The post will make the root stronger. The post won't go beyond the filling... Some filling has to be kept there otherwise the root tip may get infected again. In premolar, if it has two roots the post has to be placed in both canals(what I usually do) for better strength and retention of the restoration...
