Brief Answer:
There can be several causes... Follow up with a bit more information

Detailed Answer:
Hello,

I have gone through your query and here is my advice.

As per your history, there are several causes for swollen veins under tongue.

Please let me know do you have ulcer, fever, sharp teeth or implant, tongue bite?

For further assessment, I would suggest you to consult with ENT surgeon for detailed clinical examination. He may procure complete hemogram, vitamin B12 as per evaluation to rule out any other possibilities infection, vitamin deficiency.

Further treatment mainly depends upon the underlying condition. Meanwhile, for pain you can take tablet acetaminophen. Avoid smoking if you have it. Take balanced diet. Maintain proper dental hygiene.

Hope I have answered your query.
Let me know if I can assist you further...
