My Sister Has A Parotid Lesion - Pleomorphic Adenoma Please Advice On Whether To Have It Removed Since It May Affect The Facial Nerve

Question: My sister has a parotid lesion - pleomorphic adenoma. Smears show moderate cellularity with cohesive tubules and clusters of epithelial cells. The epithelial cells show bland nuclei. Scattered background myoepithelial cells, some with eccentric nuclei plasmacytoid appearance are noted. Focal stromal fragment is seen. No cytologic atypia is seen. We would like to get an advice on whether to have it removed since it may affect the facial nerve (7th cranial nerve). Thanks.

Hello ,
Based on the facts that you have posted It seems that your sister has developed a swelling over Paritid region which on clinical and fine needle aspiration examination is confirmed to be a Pleomorphic Adenoma.

Pleomorphic Adenoma is an benign tumour . Though it is closely in vicinity of Facial Nerve can be easily enucleated by an experienced surgeon without causing any damage to Facial Nerve .

In view of risk ( if not removed ) the chances of it undergoing changes in to becoming malignant are very high hence I would advise you to consult qualified and experienced surgeon and get it removed at earliest .

Though it is only 1.5 cm in size It is bound to increase in size over a period of time .Bigger the tumour more it is difficult to separate and avoid injury to Facial Nerve during surgery.
