Question: My mothew in law tested positive for Covid 14 days ago. She is asymptomatic. She was asked to take several blood tests , reports of which are attached. We did not do the D Dimer test. Should asymptomatic Covid positive patients be given Demamethasone tablets, and Clexene ( Clexane?) injections ?

Brief Answer:
Depends upon inflammatory markers level, and spo2 level.

Detailed Answer:
Hello
Welcome To Ask A Doctor Service
I have gone through your question, I understand your concern.
Actually if inflammatory markers like crp, d dimers are raised. And /or if spo2 at room air is less than 95 percent( in both conditions, patients may be asymptomatic).
So yes, these we initiate above medicines in many asymptomatic covid 19 patients in our wards.
I hope your query is clear now.
Thanks and regards

Dr. Biswas,
The patient test reports are attached- and you can check that:
i) CRP is 6.19, which is only slightly above the normal value of 6. Most Covid research studies I saw mentioned CRP values of 15, 18, 39 or more for severe or mild covid cases, and this seems not near that.
ii) SP02 has always been above 97
iii) Ferritin is 189, which seems in normal range.
iv) MPV is 12 , which is a bit higher than normal.
Patient has no fever and never had any. The prescription wrongly mentions fever.

Questions:
1. Given the above data, will you suggest these anticoagulant injections ?
2. Given above data, should we do a D Dimer test too, to decide about coagulation ?

Brief Answer:
dexona and clexane are not recommended as of now.

Given the above data, dexona and clexane not recommended as of now.
D- dimer should be( not must be) done.
But more importantly repeat crp and cbc should be done every 3 days, until total of 14 days completed from onset of first symptom if patient is asymptomatic.
I hope your query is clear now.
Thanks and regards
