Brief Answer:
Consult a doctor and start with the medicines

Detailed Answer:
Hi
Welcome to ask a doctor service
I have gone through your query and reviewed the attached photograph.
It is difficult to comment on the nature of insect which has caused the reaction.
It could be beetles or spider.
I would recommend you to consult a doctor.
I would recommend him to take a short course of oral steroids like tablet prednisone 20 mg once daily for 5-7 days and apply steroid antibiotic cream like clobetasol propionate and fusidic acid cream on the affected areas twice daily for 7-10 days.
This should resolve the problem.
These are prescription drugs and should be taken under the supervision of the doctor.
Also keep the affected foot elevated while lying down.

Dr Asmeet
