Question: My grand daughter 7 Year old has adenoids. She is now in Germany. When consulted ENT Doctor recently they told that Middle ear has fluid. So it is needed to remove that fluid and adenoids to be operated.
further some child specialist told adenoids will start shrinking from 7th year onwards. Kindly help and advice

Hii. Grandson is always the precious one in the world for grand parents. It seems that you are too worried about his problem. Adenoids is a gland behind the nose. It is generally enlarged in children and shrinks in size after 6-8 yr. This adenoids stay very close to eustachian tube opening. This eustachian tube is the connection between nose and ear. If the adenoids get enlarged then it will compress over the eustachian tube opening and tube will fail open to maintain pressure between ear and nose. As a result drainage of secretions will be impaired. So fluid gets accumulated in ear. This fluid can get absorbed by its own or get infected and can cause damage to ear drum, bones and can cause hearing loss. So your grandson need to do two tests: nasal endoscopy or x ray soft tissue neck lateral view and hearing tests like pure tone audiogram and impedance audiogram. For now start mometasone or fluticasone nasal spray at night. Start antibiotic augmentin and antiallergic tablet levocetrizine to prevent infection, reduce the allergic inflammation. Take steam inhalation. Continue for atleast few months. In the meantime do the hearing tests and if there is significant hearing loss and type B or type C curve is present and if adenoids is grade III then you can plan for surgery to remove the adenoids and put two myringotomy tubes in the ear drum to maintain pressure in ear and drain the fluid. So your plan should be to continue the spray and tablet and after 3 week do the other tests and if indicated plan for surgery. Review with the reports and if you have any quiet.
