Question: My fiancé has been waking up with joint pain for the past 1.5 weeks. It’s hard to say if it’s joints or muscles but it seems like it is pointing at his elbows, lower calves/ankle area. He did have blood work and a few markers were elevated. We would like you to review them and give us some insight

Hi,
I have gone through your reports.
The blood cholesterol level are on the higher side, however it won’t cause joint pain.
He has to control his cholesterol level by diet changes and exercises. Obesity can cause generalised pain especially on the lower limb.
As of now we can consider it as musculoskeletal pain rather than arthritis.
We can wait couple of weeks more to see any improvements.
If symptoms persists additional investigations like CRP, ANA profile and RA factor must be done.

Hi i thought i mentioned in the notes that he didn’t fast prior and he drank a protein shake. That may have caused the high triglycerides and cholesterol. Our concern is the protein. As you can see it’s 10.4 on the blood work and then his PTH was 1 point low ruling out parathyroid. He has been taking a protein supplement 330mg plus his protein shake has 100mg of calcium and then he was also taking L-Carnitine (i think 1000 mg). Could this have caused his calcium to be 10.4? I also read there is a relationship with calcium and albumin and you can see his albumin was slightly elevated. Can you please RE-review his blood work now that you know my concern?

Fasting lipid profile(FLP) is the best tool to assess the cholesterol status. You can repeat the test after overnight fasting to get accurate results.
Protein and calcium are on the borderline and are not significant.
It is true that albumin and protein can alter the calcium level, so that you can check a ionised calcium to get accurate calcium status.
