Brief Answer:
Advanced kidney disease.

Detailed Answer:
Hello dear, welcome to Ask a doctor service. I read your query and here is my advice.
You are having chronic kidney disease or chronic kidney failure which is the gradual loss of kidney function. This can be the cause of back pain you have.

You are having an advanced stage of kidney failure St 4 which means that kidney function is impaired and nay soon lead to kidney failure. When chronic kidney disease reaches an advanced stage, dangerous levels of fluid, electrolytes and wastes can build up in your body. You may need to use blood pressure medications to keep hypertension under control. Cholesterol levels may be increased during kidney failure so you may need to continue atorvastatine.
Using diuretics is advisable to remove fluids from body reducing swelling.
Erythropoetine may be needed to be used for anemia.
You may talk to your nephrologyst to prepare for dialysis which may soon be started.
Hope I have answered the question.
Let me know if I can assist you further.
