Posted on Mon, 2 Nov 2015
Question: A Few days ago , I had sex with a SexWorker(Most probably She is from Philippine ) in Bharian . Only for one time and after that I am in fear of HIV.
First of all I don't know if that girl has HIV or not . I kissed her and also fingering then I try to put my penis without condom , but it's not happened as it was not as hard that will go smoothly then I try to make it hard with my hand and touches the vagina Lips with penis (foreskin pull back) where vagina fluid present, and within few mnts I ejected in top of vagina with rubbing my penis without insertion in fact not get time to insert . Her vaginal fluid touches my penis top my Urethra ..

I am still in a trauma. I rubbed my penis on the lips of a vagina . Vaginal fluid touches my penis top means urethra WITHOUT PENETRATION .

Is that any chances to get HIV+. Please help me out , I am very scared. What should I do now.

I did several test,
Rapid HIV 1/2/O Antibody Only after 33 days Its showing only one line in C (with fingertip blood)
Rapid HIV 1/2/O Antibody Only after 40 days Its showing only one line in C (with fingertip blood)
Rapid HIV 1/2/O Antibody Only after 42 days Its showing only one line in C with fingertip
Rapid HIV 1/2/O Antibody Only after 45 days Its showing only one line in C with fingertip
Then on 49 days : HIV 1/2 & P24 Combo(CMIA) from LALPathLab : 0.23 Non-Reactive.
Then Day 52 : HBsAg SeRUM (CMIA) from LALPathLab : Non-Reactive
Then Day 52 : HCV SeRUM (CMIA) from LALPathLab : 0.07 Non-Reactive
Then Day 52 : HIV 1 RNA QUANTITIVE (Real Time PCR) from LALPathLab : Target not detected Copies/ml

Please suggest ... I can't Sleep,can't work .
Please help me out to understand the result accuracy and risk.

Brief Answer:
You are Free from HIV 1, Hepatitis B and C infection

Detailed Answer:
Hi,
Welcome to HCM.
Thanks for posting your query.
All the sex workers are not necessarily HIV positive. All the exposures are not necessarily infectious. 30-40% of the spouses of HIV Positive people remain HIV negative, in spite of their regular contact with their husbands till they got diagnosed as HIV positive and called as sero-discordant couples.
The test reveals the same. All the test done by you are highly reliable and from a reputed laboratory.
The nonreactive results of COMBO CMIA well after 4 weeks of exposure along with the RNA PCR clearly ruled out HIV 1 Infection in your case beyond any doubt. HIV 1 infection is common to the extent of more than 99% throughout the world except in few countries of XXXXXXX
Hepatitis B and C also had been ruled out.
HIV 2 infection is very rare and quite unlikely. Rapid test before 12 weeks are not of much value.

Brief Answer:
Suggested another test after 12 weeks before resume sex with wife

Detailed Answer:
Hi,
Welcome back.
When the doubt occurred and as you are very much concern about the health of your wife, I advise you that it is wiser to wait till 12 weeks to resume sex with your wife. There is no harm to have safe (protected) sex with your wife.
The test report will come non reactive after 12 weeks. I assure you. But seeing the test non reactive for both HIV 1 and 2 with the antibody based test, you will be more comfortable and relaxed. Otherwise any small illness would make you feel sick in future.

So, it means thr is a chances for HIV after all this test. Dr. Now I am feeling sick for thinking about it. I have to go GCC for my job where HIV test is a big issue. So I can only take a rapid test from thr as feom one of the lab where my friend is working.

Brief Answer:
All Rapid tests are equally reliable and conclusive by 12 weeks.

Detailed Answer:
Hi,
Welcome back.
Your chances for HIV 2 is less than 0.5%.
HIV 1/2/O triline test is highly reliable and conclusive by 12 weeks. Those who are steroids or other immuno-suppressive drugs or those who had ART for PEP may be likely to have extended widow period beyond 12 weeks.

Brief Answer:
HIV 2 is a remote possibility. DUO test more confirmatory by 12 weeks.

Detailed Answer:
Hi,
Welcome back.
I checked all your results sent by you. All the results were within normal limits and indicated that you are free from HIV 1, Hepatitis B and C. There is no doubt about the same. HIV 2, other than in central and west XXXXXXX rest of the world its prevalence is less than 0.5%. So it is a very remote possibility.
Even though the DUO test is reliable by 8-9 weeks and it can detect antibodies (after the window period), but considered more conclusive by 12 weeks.
