Question: Doctor is it typical following cataract surgery to have an infection start same day. There was excessive watering of the eye...I would actually say extreme. wWent back to the surgeon and he said he saw some pus in the eye. Started the antibiotic Besivance drops right away verses awaiting the follow up tomorrow. Going back first thing in the morning. Is this typical or should we be treating more aggressive? If still appears to be infection in the morning. What should be done. Multi focal lens implants

I am Dr. DADAPEER K, an Ophthalmologist and I will be answering your question.
I reviewed your history.

Infection on the same day of cataract surgery is not very common in clinical practice.
Watering on the same day of surgery can be because of surgery related inflammation and will go away. But since your doctor has mentioned regarding pus we need to rule out infection.

You need to be worried if there are symptoms like pain in the eye, redness, swelling of the eyelids, decreased vision or hazy vision...., if these symptoms are present then they indicate infection of the eye called as endophthalmitis and require treatment more aggressively.
If this condition is present the only use of antibiotic eye drop like Besivance will not be of help. This requires confirmation of diagnosis by ultrasound examination called as B scan of the eye and if confirmed treatment is by intravitreal injection of antibiotic drugs and in severe cases by vitrectomy surgery.

But I hope that this is not endophthalmitis and can be because of inflammation of the eye following surgery, in this use of antibiotic eye drops like Besivance and anti-inflammatory drugs like Bromefenac or Nepafenac will be sufficient.

Hence first thing to be done today is to consult your doctor for eye examination and to know whether it is infection or only inflammation following surgery.
If it is only inflammation then no need to be worried but if it is infection then aggressive treatment approach is required.


Do follow up with me regarding the presence of the symptoms like redness, pain, hazy vision, swelling of the eye and examination report from your doctor so that I can advice regarding the further course of treatment.

There is no redness. There had been excessive fluid...basically pouring out of his eye and running all the way down the check. He has Down syndrome and does not/ will not express pain or changes in vision. No swelling of eye lids. We used the Besivance per doctor so far 2x tonight and one more Piro to bed. Excessive tearing of eyes has stopped.

We saw the doctor. All looks good. No signs of infection. Eye is a little irritated. Possibly a blocked tear duct as well. We will deal with that after all the healing...keep a close eye on it.

Besivance 3 time a day for 7 days

Durezol 2 times a day for 7 days and then 1 I me a day for another 7

Ilevro 1 time a day for 14 days.

Revisit in 14 days unless we see any changes or pain/redness/swelling issues.

Thank you for the follow up question.

This confirms our diagnosis of inflammation, no need to be worried and he needs to continue the eye drops as advised by the doctor.

Durezol is steroid and Llevro is antiinflammatory drug (Nepafenac) and both these along with the antibiotic eye drop(Besivance) are sufficient and these will control the inflammation and he will be fine no need to be worried.
