Is Discontinuing Sotalol Safe While Experiencing Increasingly Frequent Episodes Of Mouth, Jaw Or Teeth Pain When Diagnosed With Prinzmetal Angina?
Question: I have Prinzmetal angina. This has been very intermittant over the last five years but is now becoming much more frequent. Usually around 20 times a day but only lasting around one minute each time. I only get the pain in my mouth/jaw/teeth. Sometimes I get a week or two respite before it starts again.
I have been taking Sotalol 80mg x 2 for around 12 years following Atrial Fibrillation which was cardioverted.
I have read that beta blockers are contraindicated in Prinzmetal angina and I am wondering if this has caused it.
Is it safe to stop the Sotalol after all this time?

Hello Ma’am
After going through your medical details I understand your concern and I would like to tell you that some of beta blockers like Atenolol should not be used in prizmetal or variant angina as they can increase coronary artery spasm and will cause you more symptoms but beta blockers are used in combination to treat the symptoms of Prinzmetlal Angina.
Sotalol doesn’t have any contraindications in Prinzmetal angina and I will advise you , do not discontinue sotalol abruptly as it can cause acute exacerbation of coronary artery disease .
In my opinion you should get an Angiography to rule out if you have atherosclerosis present along with vasospasm, because in presence of atherosclerosis chances of cardiac arrest increases.
Treatment will vary for both the conditions. Prinzemetal angina can be treated only with medication but prinzemetal Angina with Atherosclerotic Coronary Disease will require more medication and procedures.

As my pain from Prinzemetal angina is not very bad, lasts for a brief time and does not come with any other symptoms is it worth taking CCBs that come with side effects like swollen ankles?
