Brief Answer:
Cause of your symptoms and how to confirm

Detailed Answer:
Hi XXXX, thanks for asking from HCM.

I can understand your concern. Your symptoms could be due to

: Multiple sclerosis
: Repeat Stroke
: Diffuse cerebral atrophy
: Drug side effects as you are taking so many medications
: Paraneoplastic syndrome like leucodystrophy from recurrence of cancer

Your symptoms are significant. They must be given importance. Kindly ask your doctor to rule out above mentioned causes in you or if possible, change the doctor. The diagnosis can be confirmed by

: MRI brain with FLAIR, DWI, ADC - It will rule out old stroke, new stroke, leucodystrophy, diffuse cerebral edema and can help in making diagnosis of MS.

: Electrophoresis to know the level of M proteins for multiple sclerosis

: Drug level in body - If all other causes are negative, then drugs can be considered as one cause. The MS Contin, metropopal, trazadone combination can cause above symptoms

: Also screen for old cancer. If there is any recurrence, it must be ruled out.

: If nothing is positive, kindly rule out depression after consulting psychiatrist. It can present like any illness.

Hope it will help you. You can send me your MRI report. I will help you in reaching unto some conclusion.
If still in doubt, do let me know.
Thanks. Take care.

Brief Answer:
Thanks for appreciation and need of new MRI

Detailed Answer:
Hi XXXX, welcome back.

Thank you very much for the appreciation. As your last MRI was 6 months back, you should get new MRI brain done.

New MRI brain will be able to rule out any fresh episode of stroke, new MS lesions.

Hope you will understand it. If still in doubt, do let me know.
Thanks. Take care.
