Is Ankylosing Spondylitis Indicative Of Ulcerative Colitis?

Question: Hello, I have a history of inflammatory illness, I was with my previous rheumatologist for 29 years and in the beginning the diagnosis was polyarthritis, high sed rate, later showed high CRP. After a number of years, I began having vasculitis attacks. My rheumatologist has retired and a new dr. is reviewing my history. All of the traditional test such as ANA, scleraderma test, sjogrens ect are negative as always, they are still waiting on the HLA-b27 as I have bad back issues and have had psorosis outbreaks several years ago. My CRP is high, My C3 is high , c4 normal, c50 elevated. Do you have any thoughts on the diagnosis other than vasculitis and polyarthritis?

Thank you so much - yes the dr did test those as well. Over the years the sle test have been positive and negative so he tested again, right now negative. If the CRP is elevated (and I have a long history of elevations during flares) does the ch50 being elevated then somewhat confirm inflammation? That is a new test for me. Thank you again

Hi,

Yes, you do have inflammatory illness affecting more than one organ system.

HLAB27 would rule out Ankylosing spondylitis which is commonly associated with Ulcerative colitis which can cause recurrent diarrhea sometimes with blood.

Also, do get tested for SLE using the blood markers Anti-Rho and Anti-Smith antibodies as well as Anti -dsDNA antibodies.

This will give a clearer picture. Whatever be the fine diagnosis, you seem to need immunomodulation and need for steroid therapy to alleviate your symptoms.
