Question: My other half has a skin rash that looks like a ring worm and has been on her right thigh for about a week. It started to fade but now is getting redder and spreading in size fromabout an inch in diameter to now about 3 inches in diameter

You think your wife has ring worm or tinea corporis. This is caused by a fungal infection adn needing to be treated with an antifungal cream. Since you used the hydrocortisone the rash is gotten worse because it actually allows the fungus to grow more rapidly. I strongly recommend stopping the hydrocotisone and start using a antifungal cream like lamisil or tinactin. These will take about 2-4 week but will heal the problem.

In summary
1. ring worm is a fungal infection
2. Steroids like hydrocortisone makes the rash worse
3. Stop the hydrocortisone
4. Start Lamisal or tinactin cream 2-4 weeks

Tinea corooris is contagious but not highly contagious. You should change your sheets like once a week while undergoing therapy. No other precautions are needed

After an incremental decrease in itching over night while treating with Tinactin spray, my other half woke up with an increase in redness and further discoloration approaching the brown color spectrum at the core. We suspended the Tinactin and found some 2% ketoconazole cream which seems to be effective. What further instructions would you have? Would you recommend cleaning the affected area with apple cider vinegar before each application of the cream?

Ketoconazole a good powerful antifungal

With the Ketoconazole your wife's rash should respond quickly. There really is no need to add apple cider vinegar to the area it will not change the outcome of the treatment

Ketoconazole is a much stronger and more broad spectrum antifungal and should easily cure the rash
