Question: Hello doctor my baby of 30 days finds hole of 2.55mm in asd /VSd(posterior) /moderate PAH..it will automatically closed or operation needed..

Small ASD of size less than 4 mm gets closed with time in almost 100% cases.
Similarly small VSD(perimembranous or muscular) also get closed as the baby grows and gains weight.
Large VSD,subpulmonic VSD or malaligned VSD are less commonly closed by itself.
Pulmonary arterial hypertension(PAH) can be there in normal infant till 6 weeks of age.This pulmonary hypertension is usually does not need any treatment.
Most of the babies with VSD usually gets symptomatic(symtoms of heart failure like rapid respiratory rate,incresed sweating,difficulty in feed etc)by 6-8 weeks of age when this physiological PAH gets decreased.The medical treatment is needed at that time only depending on the symptoms.
I would request you to attach the reports so that I better able to understand the size and position of VSD.
I agree with your doctor and would like to get the repeat ECHO done at age of 2 months to check whether the pulmonary hypertension has regressed or not.Depending on the echo report during follow up the decision of medical treatment or surgical closure can be done.
