Question: I just started on methotrexate and have MTHFR genetic issue with folic acid metabolism. What would be the best way for me to supplement with folic acid? Is it necessary for me to use metholated folic acid.

Yes, methylated folic acid or 5-MTHf is considered to be superior compared to simple folic acid. Various studies have shown that 100mcg of both in MTHFR deficient people, methylated folic acid was of more efficacy and of lesser side effects but due to still lack of data, many countries and their drug authorities have not proposed the exact guidelines and thus leading to unavailability of these in many countries.

You can discuss it further with an endocrinologist as case to case variations may occur. So he will be able to prescribe after carefully looking for any contraindications to use methylated formulations in your case.
