Is HIV DUO Ultra Test Done 6 Months After Exposure Conclusive?

Question: Hello Doctor - i had a possible exposure 8 weeks back - i had protected vaginal sex with expired condom, but was sure i did not come inside my partner - because when i ejaculated it was inside condom, i have unprotected oral receiving sex from my female partner (whose status i dont know), i had HIV DUO ULTRA TEST on 5th Day, 7th Day, 11th day, 31st day and post 6 weeks - all of these HIV Duo test was Non reactive / Negative - should i be worried about HIV Risk - please help me with an answer

Please find my observations below.

1. The latest test for HIV testing which is recommended internationally is duo test which is the rapid 4th generation HIV test for HIV 1 and HIV 2 antibodies and p24 antigen. This test is conclusive for HIV 1 infection after 30 days in countries like the US where there is only HIV 1 infection.

2. However because you are in XXXXXXX therefore there is a 2 percent risk of HIV 2 infection. Therefore the test for HIV 2 antibodies is to be done after 6 months.

3. There is no separate test for HIV 2 infection and you can get tested for HIV 1 and HIV 2 antibodies after 6 months from exposure for confirmation. This is because HIV 2 infection has maximum window periods of 6 months and there is no other test which can look for this infection.

3. There is no need to have mental tension because 6 weeks duo test is negative and this means 100 percent that you do not have HIV 1 infection. However there is a 1 to 2 percent chance of HIV 2 infection

5. If you do the duo test 6 months from exposure and if the result is negative then it will be completely confirmed that you are safe from HIV infection.

sorry sir im bothering you much, may be this is my last question, not able to get a peaceful sleep for a long time that is why.

When i had sex with my partner, there was a moment i stopped having vaginal sex and after few mins i ejaculated at that time i was not even near her and i noticed throwing away the condom without any breakage with semen into the bath room

This oral sex where i was a the receiving partner happened for a brief moment like 45 seconds or so. and also post this event i had never had any sexual interaction with anyone.

I will take you advise of doing a retest after 6 months, but can i just think this is a minor risk and for major part im cleared and move on with my life - please help me sir - thanks and sorry for bothering doctor

1. The problem of increased risk for you to develop any infection is not from ejaculation. Ejaculation is a risk for pregnancy and for disease transmission from you to her. I guess you do not have any sexual infection from before therefore the concept of you ejaculating does not come in to the picture when discussing infection transmission to you.

2. Having sex without ejaculation is still having low risk of HIV transmission from her to you. This risk is less if you used condoms however the condom was expired and we cannot rule out any vaginal fluid leaking through the condom during sex.

3. The oral sex is very less risk for HIV infection.

In conclusion, the risk is extremely low and I have suggested testing for peace of mind and to get conclusive report.
