default
Posted on Tue, 24 Mar 2015
Question: Hi,
I accidentally had sex 12 days back with condom.but it broke at the end .I dont know the hiv status of the lady and Iam scared. It was jus my first time and it went wrong.Later on I have gone through the internet and came to know that hiv dna PCR test will give accurate results after 14 days .
Is it possible to get an accurate result with this test and is there any probability that this test might give false positive.
May I know what all tests to be done to know that Iam hiv free. Will Pcr test is enough to confirm or do I have to under go any other test after 30 days of possible exposure.I read that hiv can b detected after 30 days.Could you please walk me through all the tests and stuff that I shud take to confirm.
If possible could you mention the best lab where can I get tested in XXXXXXX

Find your answers

Detailed Answer:
Thanks for your question. I am Dr Matin and will be answering your question.
Here are your answers:
1.The risk of getting HIV through sex with broken condom with an infected partner is equal to that of unprotected sex .Since the status of the partner was unknown ,so probability will depend on the prevalence rate of HIV in that area too.
2. HIV DNA PCR qualitative assay can not be used as a screening test or a primary diagnostic test for HIV-1 infection except in infants of <2 years of age born to
HIV-1-infected mothers.
3.There are other tests -Antibody tests(3rd Gen) , Antibody +p 24 antigen test(4th Gen), Western Blot test and Nucleic acid amplification test (NAAT).
4. In your scenario , NAAT can be advised which provides the earliest indication of HIV infection,Using these approaches Now it is possible to reduce the period of time to diagnose an HIV infection from approximately 22-24 days to 11-12 days (nucleic amplification tests (NAAT).
5.In any case whatever the test you go for , it has to be confirmed by another any HIV test after 3 months of exposure to be absolutely sure of one's status -positive or negative.
You may go to a any Govt run Integrated Counseling & Testing Centre (ICTC) in XXXXXXX under NACO (National AIDS Control Organization) , where you will be counseled first ( to assess your previous knowledge about HIV , meaning of positive or negative results, to assess your possible risk and to take your consent for testing), following which your blood sample will be tested thrice simultaneously and then your result will be communicated to you after another set of counseling. Counseling gives you a cushion to assess your risk, to know about the illness and to equip you face your challenges.
Hope this is useful

Brief Answer:
Please read below to get your answers

Detailed Answer:
Thanks for your response. The sensitivity ( power of picking up positive cases in HIV positive cases) is in the tune of 97%.
It can be taken with any other test.
Whatever be the result , you have take another test post 3 months of your exposure as per protocol to be absolutely sure of your status.
Not sure about the COST OF THE test or its availability in Lal pathology lab, you can enquire there about it.
Hope this is useful

Brief Answer:
You can go for 4th gen test

Detailed Answer:
You can go for 4th generation test (which tests both --antigen p24 and antibody) p24 is found raided only in early phase of HIV , so helpful in diagnosing early cases.
This test is widely available in any good lab and is equally useful in diagnosing early cases of HIV .
But whatever be the result, you will have to undergo another HIV test @90 days of exposure as per protocol.
Hope this helps.

Brief Answer:
The results are trustworthy

Detailed Answer:
Yes these tests are ok and the results are trustworthy .There are no data available to predict your chances at 16th day of exposure with these reports.
You have to go as per protocol that is next test at @3 months of your exposure.

Thanks
