Brief Answer:
Fentanyl 50 can be reduced

Detailed Answer:
Hello and thank you for asking

It is good news that chemotherapy had good results in terms of disease progression

But now as you said after pancreatitis he developed pain. I believe he has taken the medication for the pancreatitis

One of side effects of fentanyl its sleepiness, so in my opinion if he have such side effects the dose can be reduced to 25

Feel free to ask further
