Brief Answer:
Lower respiratory infection most likely.

Detailed Answer:
Hi

Thank you for asking HCM.

I have gone through your query. Your problem can be most likely lower respiratory infection. The high ESR shows its severity. Antibiotic therapy , bronchodilators , mucolytics and proper rest for 1 week will be helpful to normalize your condition.

Take warm fluids. You can take warm chicken soup with pepper, lemon tea , and chewable vitamin C.

If you getting cold and upper respiratory infections then an ENT consultation should be done to rule out chronic sinusitis or tonsillitis. Taking vitamin C daily will be helpful also.

Hope this may help you. Let me know if anything not clear.
Thanks.
