I read your question and i understand your concern. Haemorrhoids are engorged cushions and need to be treated with not just hydrocortisone, but complete diet lifestyle adjustment, and treating underlying cause which is mostly constipation at this age. Treatment also depends on grading and severity of hemorrhoids, if those are grade 3 or higher, then conservative measures wont suffice and you may need hemorrhoids surgery via minimally invasive approach .

1 % hydrocortisone cream topical if not working can be considered for hydrocortisone suppository, add to it warm sitz baths with povidone and saline water , use high fiber diet, take laxatives for constipation, adapt good bowel habits, and still if the response is not good you may add 2.5 % hydrocortisone to the dose. Talk to your general surgeon and let them decide what is best for you.
