In What Way Can I Manage Severe Gas And Acidity After Completing GERD Medication Course?

Question: Hi Doctor,
I had GERD in 2015 and Completed Course suggested by Local Gastro Dr. After that Food coming back to mouth stopped, but now a days getting severe Acidity and Gas. I need to keep on taking PAN 40 daily, earlier I have taken esomac d 40mg. - Taken for 4 Months. But as a side effect now I can not eat anything with red chilli powder or Acidic Like Orange etc. If I ate that up, next day I will have sever Acidity and Gas with pain near chest area. I had been run to hospital for many times due to False Heart Attack, it was merely Acidity Attack.
Even Currently also I have severe Gas and bloating, planning to take unienzyme to relieve gas symptom, but why this GAS and Acidity keep happening, though I am following very light diet - without oil and Chilli or chilli powder or anything acidic

Hello,

Have seen your details and i appreciate your concerns,
As per your query i would like to tell you that GERD usually requires life long treatment however medicines are tapered off as per the response of patient to symptoms.Our gastrointestinal tract has a forward propulsive movement (peristalsis) that moves the food material forward as it is digested, absorbed and finally expelled as feces. If somehow this movement gets hampered, food along with the digestive juices would stagnate, stomach acid will reflux into the food pipe this causes severe acidity and heartburn and on the other hand the bowel habits would not be proper. Degradation of undigested food in the intestines by the intestinal bacteria would lead to the excess gas formation.

So,
See as the name suggests it would be treated in two phases-
1)Control production of acid-For which a h2 blocker or ppi can be taken when ever you take spicy food double the doses.
2)To prevent the acid formed inside stomach to reflux back into esophagus--I would suggest you to add Levosulpiride instead of Domperidome. It's a excellent prokinetic drug and is very effective in controlling reflux of acid back into esophagus.

Life style changes and food modification are a part of treatment however in most of the cases depending on response of patient low or high doses of these drugs are required.
Secondly have a endoscopy done on you may be you have ulcers in stomach or intestines?
