I'm Re-asking My Question As My First Response Disappeared. In

Question: I'm re-asking my question as my first response disappeared. In 2002 my body collapsed, with extreme fatigue and weakness leaving me non-functional. I've been diagnosed with CPSTD due to childhood emotional abuse and marital emotional abuse (50 yr marriage).
I've learned I was misdiagnosed with PsA and treated for that for 14 years. I had triple negative breast cancer in 2016, melanoma in 2007, 2012 & 2017. I am now diagnosed with Fibro/Chronic Fatigue. Many of my symptoms were never addressed by MDs. I've always suspected I have thryroid issues as every other woman in my family has had them, but test results were always normal. My life is so restricted at this time, I'm determined to find the cause and recover what I can of a normal life. Looking at all symptoms, any idea in what direction I should proceed? Thanks, XXXXXXX XXXXXXX

chronic fatigue syndrome

Detailed Answer:
Hi...
Welcome to "Ask a Doctor"service.
I am Dr Ajeet XXXXXXX

I have gone through your query and here is my opinion.

The symptoms which you have narrated may be due to a chronic infection, hormonal imbalance or mental disorder.

If the infection part has been ruled out by the clinical examination and relevant investigations like blood counts, the hormonal imbalance should be excluded.
As your thyroid profile is normal, the possibility of an adrenal burn out is there, which can be evaluated by blood work.
If all the investigation are normal, there may be an endogenous/reactive depression or some stress disorder.
Difficulty in managing stress may contribute to the development of chronic fatigue syndrome too.

There are no simple blood tests or scans to diagnose chronic fatigue syndrome( Myalgic encephalomyelitis).
It is a diagnosis of exclusion, based on the symptoms which includes extreme tiredness which is inappropriate for the amount of physical activity and which is not relieved by rest or sleep.
There is feeling of physical or mental exertion, difficulty in concentrating, memory issues, dizziness, lightheadedness, blurred vision and other symptoms which are non specific and make the diagnosis more difficult.

Your doctor must have ruled out other diseases mimicking CFS before diagnosing chronic fatigue syndrome.
These may include Sleep disorder.,Anemia,Diabetes, hypothyroidism, heart failure, deranged cortisol levels, depression, anxiety, bipolar disorder and schizophrenia.
If all the organic causes (as above)have been excluded,I would also keep CFS as my first differential diagnosis in such a patient.

Kindly share your investigation reports, your genetic history, personal history and other symptoms of yours in addition to fatigue. How is your sleep pattern?

I would advise you to make some dietary and life style modifications.

Take high fiber diet.
Eliminate sugar, processed food,Junk foods from your diet and eat plenty of raw fruits and vegetables.
Avoid alcohol and caffeine.
Do graded exercise.
Get a good 8 hours night sleep.
Learn relaxation techniques and meditate.

Stay healthy.
Regards.
